Depression Questionnaire: Which of the following is true for you?

You find yourself missing more work than you used to, though you know it's not because you're "sick."

Things that used to give you pleasure don't seem very enjoyable anymore.

You're having trouble concentrating.

You have little appetite or you're overeating.

You're sleeping too much or too little.

You feel tired most of the time, but your doctor can't find anything "wrong" with you.

You have trouble sleeping because either (A) it's hard to fall asleep, (B) you keep waking up, and/or (C) you wake up very early and can't get back to sleep.

You feel as if there's nothing you're looking forward to.

Sometimes you think people would be better off without you.

Your relationships are suffering, but you don't have the energy to work on them.

You've thought about ways to kill yourself.

 Your sexual interest has diminished.

If you're depressed, it's crucial to find help. Depression is treatable. If you're suicidal, please call the crisis hot line, 512/472-HELP (4357), in Austin. Elsewhere, contact local information and ask for the nearest crisis center, or get General Mental Health Information from 800/789-2647.
