Internal Hazards Survey and External Hazards Survey

This form is intended to allow a systematic inspection of an institution and its surroundings. Surveys can be an instrument of prevention in allowing areas of risk to be identified and eliminated, or if that is not possible, regularly checked.

Internal Hazards Inspection Checklists

Once specific hazards have been identified, list them for regular inspection. The list should include not only the hazards, but also the person(s) responsible for checking them, and the date of inspection.


INTERNAL HAZARDS SURVEY
* Hazardous Condition

Building General

* Internal finishes or furnishings which burn easily (dry or highly varnished wood imitation wood veneer paneling, etc.).

* Interior finishes or furnishings which give off toxic smoke when burning.

_______

* Large, open spaces, high ceilings with concealed spaces.

_______

* Windowless areas which would make fire fighting access and smoke
removal difficult.

_______

* Interior basement walls treated with water-resistant sealer before
painting; resealed periodically.

Compartmentation and Fire Protection

Fire walls and fire doors around vertical access areas and equipment
rooms.

_______

Collections areas separated from other areas by fire walls and doors.

_______

Fire doors closed and unobstructed.

_______

Internal book drops reinforced with fire-resistive material or
otherwise protected.

_______

Fire exit signs visible and illuminated.

_______

Fire detection and suppression systems regularly inspected and
maintained.

_______

Detection systems connected directly to fire service.

_______

Fire fighting equipment accessible.

_______

Sufficient fire extinguishers of proper type, charged and inspected
regularly.

Electrical, HVAC, and Plumbing Systems

Electrical and plumbing systems regularly inspected and maintained.

Emergency lighting operable.

_______

Ventilation ducts clean.

_______

Automatic fire dampers (metal) on ventilation ducts.

_______

Circuit breakers and electrical controls accessible; locations marked.

Water cutoffs accessible and locations marked.

_______

* Water pipes or drains running through collection areas.

_______

Collection environment stable.

Collections and Housekeeping

_______

* Collection materials stored on floor.

_______

Collection materials returned to proper housing after use.

_______

* Books shelved improperly: packed too tightly, or hanging over edges
of shelves.

_______

Nitrate negatives in photographic collections stored separately and
properly.

_______

Aisles, exits, and stairwells unobstructed.

_______

Trash, cleaning rags, etc. disposed of properly and promptly.

_______

Combustible trash placed in metal containers.

_______

* Electrical extension cords over six feet; cords run under carpeting.

_______

Supply closets and slop sink areas orderly.

_______

Supplies stored separately from collections.

_______

Solvents and other flammable materials stored in marked containers in
cool, safe place out of sunlight and away from heat.

Smoking prohibited.
